New forms were shipped in mid-March for the Iowa Newborn Screening program. The new forms are identified with an expiration date of "2/2009" on the left hand tab.

Effective immediately, use only the new forms for newborn screens.

Please destroy all of the old forms (those with an expiration date of "3/2007").

Effective immediately, the Ankeny-UHL Newborn Screening Laboratory will accept only the new forms and will reject any specimens collected on the expired forms.
