The University Hygienic Laboratory (UHL) has identified inconsistent use of current test forms as a significant factor in slowing the accessioning and reporting of test results. Your assistance in improving this process by utilizing the correct form is greatly appreciated.

The new form is to be used for all serology testing (including West Nile Virus) with the exception of hepatitis. Hepatitis is part of a study with Iowa Department of Public Health (IDPH) that requires a separate submission form.
