Family Medicine Inpatient Admission Rules

The Family Medicine inpatient service exists to provide excellent care for the patients of our medical home to the extent that we can safely do so and to provide a robust and varied educational experience for our learners. We care for patients of all ages with all types of problems. There is a separate Maternal/Newborn Family Medicine service that provides care for all of our pregnant patients and infants less than 3 months of age.

 

Admit the following patients to Family Medicine, unless at CAP:

1. Established patients of Family Medicine. These are patients seen at least once by a UI family physician/provider at one of the UI FM clinics (UIHC, Iowa City-Scott Blvd, IRL-East, River Crossing, North Liberty, Muscatine clinics) within the last two years and based on the admitting team is the patient likely to follow-up with a UI FM provider. (UI Quick Care/Urgent Care/Cedar Rapids visits and other distant non-Family Medicine Department affiliated clinic visits do not qualify.)

2. UI varsity athletes with a medical diagnosis. 

Note: For Patients < 3 mos, the Family Medicine staff on Mother/Baby call will be placed on the bridge for the admission.

 

Is the Family Medicine inpatient service at absolute or surge CAP?

Yes—at CAP: the Family Medicine faculty or senior resident/8096 on call will contact ATC staff to go "on" and "off"  CAP. This is needed because the EPIC Family Medicine inpatient list is often inaccurate. The Family Medicine staff will instruct the ATC staff to contact instead the appropriate Internal Medicine triage staff: General Medicine triage or Cardiology teaching/CVICU triage who will admit the patient. If the Internal Medicine triage staff has questions, they will contact the Family Medicine faculty physician on call.

No—under CAP: ATC staff will contact the Family Medicine staff and senior inpatient resident on call who will bridge the admission.

 

Family Medicine Absolute CAP (16 patients on service):

1.      The senior resident and faculty member are responsible for determining the number of patients on the service.

2.      Patients “on the service” include all patients on the FM EPIC inpatient list AND inpatients in the OR or endoscopy suite AND other patients in the ED and clinic who have been triaged with the team and are awaiting admission. It does not include long term "boarding" patients.

3.      Exceptions to the CAP: pediatric patients (under 18 years old), “bounce-back” admits within 5 days (for any admission diagnosis) and patients admitted from the UIHC Family Care Center are always admitted to Family Medicine even if the service is at the CAP.

 

Family Medicine Inpatient Surge CAP (4 admissions/shift): 

The family medicine service will admit 8 patients in a 24 hour period, with no more than 4 admissions per shift.

The shift is defined by the hours of the supervising senior resident on service.

 

 

 

Shift Hours:

1. Weekdays (M-F): 7 AM-6PM, 6PM-7AM

2. Saturdays/Holidays: 7AM-7PM, 7PM-7AM

3. Sundays: 7AM-830PM, 8:30 PM-7AM

The family medicine inpatient service will continue to admit patients that meet the same above criteria for exceptions to our service CAP even if the surge cap is met.
