Application Checklist

• Complete an application to the program.

• Official transcripts from high school and all post-secondary educational institutions.
• Four personal reference forms, using the standardized form on the application form's page, one completed by a radiation therapist and three others by individuals such as supervisors, educators and co-workers that are able to describe your characteristics. References should be capable of addressing all or most of the characteristics listed on the reference form. To ensure confidentiality, each recommendation must be individually sealed in an envelope which bears the signature of the person writing the reference across the seal. Personal letters of reference are not accepted.
