Instructions for Shipping Cord Blood
Dear Sir or Madam:
Thank you very much for your interest in donating cord blood to the University of Iowa Stem Cell Bank.
Here are some instructions that can help you when shipping the cord blood:
- There are three consent forms. The patient should print her name, sign and date all three. One copy is for the patient to keep, one copy is for the OB to put in the patient chart, and one copy is sent back to us for our records. You may return this copy in the box along with the cord blood sample.
- The instructions for the clinician to collect the cord blood are included in the kit. Please give the kit to the clinicians assisting with the delivery; they will collect the cord blood. They should also attach a sticker with the patient name and time of delivery to the bag.
- Once the cord blood is collected, it needs to be shipped to us immediately; cord blood viability is very limited. The cord blood sachet and 3rd consent go inside the Styrofoam cooler inside the cardboard box, the sealed box is shipped back priority overnight FedEx at our cost using the pre-filled form. The box may be dropped off at any FedEx shipping office using the above mentioned shipping form.
