The Surgical Pathology Laboratory receives all biopsy and surgical resection specimens obtained from University of Iowa Health Care patients, as well as cases referred from other pathologists and cases for review before a patient is seen at University of Iowa Health Care. In 2004, 27,027 patients’ cases were accessioned into the laboratory. The faculty and fellows determine if any part of the specimen needs to be routed to other laboratories for examination. The Surgical Pathology technical staff assign accession numbers, label the specimens, and label cassettes for identification purposes. The Pathology faculty, fellows, residents, externs or technical staff dictate a gross description of the specimen, perform any necessary dissection, and chose the appropriate amount and/or location of sampling needed for histologic evaluation. They also determine the proper fixatives and fixation time for each specimen. The number of tissue blocks is entered into the laboratory computer to notify the Histology Laboratory of the number of cassettes from each case. The technical staff load the tissue into machines for automated, overnight processing. Paraffin embedding, sectioning onto glass slides, and hematoxylin/eosin (H&E) staining are done in the Histology Laboratory when processing is complete.
A fellow reviews all cases from the Histology Laboratory and records a preliminary diagnosis as well as alerting the laboratory of any quality problems. The residents and externs perform a microscopic examination and dictate a description and diagnosis. The histology slides and dictated reports are reviewed with faculty who correct and/or amend as necessary. The cases return to the Fellow for Quality Assurance evaluation, and are sent on for final typing and approval by the faculty who reviewed the case.
The Surgical Pathology staff are also asked to perform intraoperative examination of some specimens when an evaluation is needed to make an immediate decision about the surgical procedure. These specimens are frozen to make them hard enough to cut a thin section for microscopic examination. An examination of this frozen tissue section is done and the findings reported back to the surgeon in the operating room.
