Rejection Criteria:

The laboratory will only accept the Hemoccult card with the specimen 
applied.  Stool specimens will not be accepted and will be returned to 
the sending area.

24 hrs/day, 7 days a week, including holidays.

Comments:

This test is offered in the Core Lab (6240 RCP) and the Family Care 
Center Lab (01250 PFP).  The laboratories will not stock the card, only 
the reagents to perform the test.

Expected TAT:  20 minutes

Please print, complete and submit the Advance Beneficiary Notice 
(ABN) along with the Laboratory Requisition before shipping the 
specimen.

Place requisition into outside pocket of bag.
Ship at ambient temperature.
