Lavender top tube (EDTA)
Minimum:
5 ml blood in lavender or yellow top, 1 ml bone marrow aspirate in 
lavendar or yellow top, or 3 cu mm of fresh frozen tissue.  Blood and 
bone marrow must be fresh; separated mononuclear cells may be frozen.

DNA extracted from blood or bone marrow mononuclear cells or tissue is
examined for rearrangement of immunoglobulin heavy chain genes,
immunoglobulin kappa chain genes, T cell receptor beta chain genes.  
Test is run weekly.
Methodology:
Southern Blot

Processing:
Do Not Centrifuge.
Indicate specimen/fluid type on requisition.
Invert tube gently several times to mix blood.
Label transport tube with two patient identifiers, date and time of 
collection.
Maintain sterility and forward promptly to lab.
Submit whole blood in original container.
Specimen source is required on requisition for processing.

Blood Specimen - Room temperature.
Tissue Specimen - Freeze immediately and store frozen.

Instructions:
Place requisition into outside pocket of bag.
Recommend express mail or equivalent if not on courier service.
Instructions:
Tissue samples:  Freeze specimen immediately. Transport on dry ice.
CPT Code:
83890, 83892, 83894, 83896, 83912; if paraffin block or fresh tissue is 
submitted, remove CPT code 83890 and add 83907.
