Comments:

Patients/donors must be scheduled in advance.  Send E-1 UIHC 
Consultation Form and Physicians Request Form to DeGowin Blood Center -
Donor Collections area (C250 GH).  This program is reserved for
patients who are medically unable to donate blood for themselves for a
pre-scheduled elective surgery.  This program cannot provide units on
an emergency basis.
