Requisitions must be in the laboratory by 2400 for next morning phlebotomy rounds or by 1700 for evening phlebotomy rounds. Requests for phlebotomy after these deadlines are subject to the guidelines below.

Adding Tests to patients who already have scheduled AM or PM draws

Please leave a completed requisition in the Laboratory folder with "ADD-TO" written at the top of the requisition. The requisitions must be available for the phlebotomist by 0600 or by 1730.

Adding Patients to Morning Phlebotomy Rounds

1. "NIGHT SHIFT LATE ADMITS" or patients who are admitted up to 0200: Send requisitions to the lab for the AM draw.

• Write "LATE ADMIT" at the top of the requisition.
• Call lab and tell them you are sending a requisition on a newly admitted patient.

2. Draw requests AFTER 0200

• Fill out a requisition and write: "ADD-ON - AM Draw" and the date at the top.
• Keep the requisition at the unit desk and place it in the Phlebotomy Labs folder.
• Fill out a Phlebotomy Add Sheet (titled: Nursing Collection Checklis ) found in the PHLEBOTOMY LABS folder at the nursing desk. If you cannot find this form, the lab will tube a copy to you. (call 6-3527).
  • Place an IPR Barcode sticker on the Add Sheet and record the ROOM/BED and ALL requested tests for each additional patient.
  • Deliver the sheet, by pneumatic tube or hand, to the laboratory before 0500 AM.
  • A phlebotomy supervisor will call you at approximately 05:45 AM if staffing is NOT sufficient to draw the additional patients.

3. You may call #6-4548 after 0530 to speak with a phlebotomy supervisor.

Adding Patients to Evening Phlebotomy Rounds

1. Call all late requests (after 1700) for evening phlebotomy draws to Specimen Control (6-3527).

• Give the laboratory personnel the
  • Patient care area
  • Name of patient
• Place requisitions in the Laboratory Folder on the nurses station desk.

2. Phlebotomy will make a second round when all scheduled draws are completed. Phlebotomy will NOT draw unscheduled patients during their first round.

Exception: Phlebotomy will complete any additional patient draws on the Pappajohn psychiatric floors while completing scheduled orders.

NOTE: If phlebotomy is unable to complete all late phlebotomy requests, the laboratory will call the inpatient units by 2100 to tell them that Phlebotomy will not return.
