Written Informed Consents are required to add tissue samples to our Repository. Please complete the appropriate form(s) below:

Please click here for a conset form if a biopsy is being planned for research purposes only.
In addition, if the clinical/research subjects are aged 7-12 or are cognitively impaired, the appropriate assent forms must also be completed.
