UI Hospitals and Clinics - Maintenance and Environmental Services   Maintenance/Environmental Services requests or trouble reports can be made by dialing 800 on any in-house telephone.   Maintenance/Engineering Requisition   To request a quote for work, send email to Dale Flannery. To open a form to submit a Maintenance/Engineering Requisition to the Work Control Center, click here. This form can be printed and faxed to the number indicated on the form.   Hospital Moving Requests   To submit an online Hospital Moving Request, click here. One week prior notice of the move date is requested. If there are questions, please call the Environmental Services office at 6-2277.   Surplus   Environmental and Guest Services no longer will be able to accept surplus request forms from hospital departments. Please visit the UI Surplus website at www.uiowa.edu/uisurplus and use the new online surplus removal request form.   Cleaning Request   If you have a room or rooms that needs unscheduled cleaning or needs project cleaning (wall washing, carpet extraction, window cleaning etc), please open the Environmental Services Cleaning Request and fill in the requested information. Be sure to fill in the name of the department head. A twenty four hour notice is requested. Send this to the Utility Supervisor via e-mail by clicking on the “submit” button.
