ADMISSION ALGORITHM

1. Was the patient discharged from an inpatient Internal Medicine or Family Medicine team in that same calendar month? If yes then that same team readmits the patient. If no, then see below.

2. Was the patient’s most recent primary care visit been by a UI general internist or family physician outside the PCCN within the last two years?
Yes: admit to that doctor’s inpatient service no matter what service is on no doc call.
No: Has the patient been seen by UI specialty medicine services routinely in the last two years. If yes, then admit to Internal Medicine. If no or if most recent primary care contact was in the PCCN then the patient is a “no-doc” and goes to that day’s admitting team.

3. If the first on call staff physician called by admissions/bed placement staff defers the patient to the other team that physician must communicate their availability to be called back by their other on call staff colleague if there is confusion.
