Dealing with health insurance can be complex. Please

Before your appointment:
- Your costs and your insurance coverage
- Getting pre-approval for costs through your insurance company
- Payment options if you do not have insurance coverage

who can tell you more about potential state and federal programs to help
with expenses.
Remember:
- If you decide to use more services than you originally requested,
you may be charged an additional amount for those services.
- Even though your insurance company said it would approve the visit,
it may not covera all -- or any -- of the costs.
- You have the right to appeal any decision made by your insurer about
what it will or won't cover.
You are the person responsible for seeing that your bill
is paid. If you have questions about your bill or if you want to set up
