Paraffin-embedded, formalin-fixed tissue block at 20-25 degrees 
celsius.  Protect paraffin block from excesive heat.  Ship in cooled 
container during summer months.

If paraffin block is unavailable to send, submit 12 unfixed tissue 
sections on 'plus' charged or silanized slides.
